Persistent primitive hypoglossal artery with atrial septal defect.

چکیده
A 66-year-old man was referred to our department for investigation prior to cardiac surgery on ASD. He had no complaints. On neurological examination, there were no abnormal symptoms. ASD was reconfirmed by ultrasonic cardiography and cardiac catheterization. Cardiac catheterization showed that the pulmonary-to-systemic flow ratio (Qp/Qs) was 2.49 and surgery was recommended. Because magnetic resonance angiography (MRA) showed an abnormal artery, catheter angiography was performed. Angiography revealed the abnormal artery left right ICA to be a large extracranial branch and the basilar trunk originated from the artery. CT revealed the right anterior condyloid foramen to be enlarged (Fig. 1). Therefore, the abnormal artery was thought to be PPHA. Cardiac surgery was successfully performed.

BACKGROUND Persistent primitive hypoglossal artery (PPHA), a remnant of embryonal circulation, is a rare variant of the posterior cerebral circulation. Seven prior cases of posterior circulation stroke in the setting of PPHA have been described in the literature, with all but one case being attributable to atherosclerotic embolization from the internal carotid artery (ICA) through the PPHA. C...

Herein, we are presenting a case of persistent interatrial septal defect diagnosed during coronary artery bypass grafting (CABG). Twice, attempts had been made to close this shunt using amplatzer septal occlude. However, percutaneous technique had failed in both occasions. The patient presented with chest pain 4 years after the second attempt and required urgent CABG. Persistent shunt was repai...

An elderly male was evaluated for subarachnoid hemorrhage and found to have a persistent primitive hypoglossal artery with its associated abnormal posterior fossa circulation and a lower basilar artery aneurysm. Angiography findings are described and are corroborated with computed tomography and magnetic resonance imaging. Its surgical and clinical relevance are discussed.

Atrial septal anomalies suggested to play important role in enhancing tendency to induce cerebral embolic events. This study was undertaken to find the frequency of atrial septal anomalies including Patent Foramen Ovale, Atrial Septal Aneurysm and Atrial Septal Defect in patients with etiologically unknown stroke categorized as cryptogenic.Materials and Methods: 32 patients were studied by tran...
